Welcome to Agassiz Village 2017!  We are very excited to have you as part of our summer camp staff.  This welcome and employment packet has a lot of information that will help you understand what Agassiz Village is all about.  It will also assist you in your understanding of staff policies and procedures, and answer some of your questions about your position at Camp this summer.

This welcome packet contains the following items that require your immediate attention: AV Personnel Policies & Procedures, a Background Check Consent, Health History and Physical Exam forms, and a W4 tax form. 

In order for us to confirm your employment at Agassiz Village this summer, we need to receive the following forms back from you by 05/31/17: Offer Letter (sent to each staff member by email), Policies Receipt, Background Check, and W4.  Health forms can be turned in at a later time.

  1. Personnel Policies & Procedures Receipt - Please read the Personnel Policies & Procedures (see the link above).  Download, complete, SIGN and return this receipt that states you have read, understood, and agree with them.
  2. Health History & Physical Exam Forms (2) – These forms (2) contain vital information about your health status. Please complete the Health History Form yourself and have your doctor complete your Physical Exam Form and return it to us. A current tetanus shot (in the last 10 years) is mandatory.
  3. Background Check Consent - All staff members are required to undergo backgrounds checks.  Please download, complete, and sign the Background Check Consent Form and send it back to us. 
  4. W4 Form - A W4 form indicates your tax exemptions and your social security number, enabling us to determine the amount of taxes, if any at all, to be withheld from your paycheck. Please complete the form appropriately and return it to us.

Please remember that until all necessary forms are returned to us, you will NOT be confirmed as an Agassiz Village staff member.  For HR related paperwork, please be sure to bring with you to camp at least  TWO FORMS OF IDENTIFICATION  (unless it is a passport, in which case you’ll need only one).  The two forms of I.D. could be your Drivers License and Soc. Security Card, etc.

Please be aware that we will be doing one pick-up in Boston on June 25 (an exact time and location to be determined at a later date).  If you cannot make the pick-up, you will have to arrange your own transportation to camp.  All staff must arrive by 5 p.m. on June 25, 2017.  If your start date is other than June 25, please refer to the Travel Information Document for further details.

  5. Parental Consent – All staff members under the age of 18 are required to obtain permission from parents/guardians to work at a residential site.  Please have your parent/guardian sign the form and send it back to us along with the other forms.
